Nurse Practitioner

Mass General Brigham is seeking a skilled Registered Nurse (RN) to provide high-quality clinical services to patients in various settings within the community. As a member of our interdisciplinary team, you will deliver patient care that aligns with our organization's philosophy, policy, and standards of community health practice.
Responsibilities- Provide skilled nursing care to patients in various settings, including assessment, teaching, treatment, documentation, and care coordination.
- Perform admissions process and case management responsibilities in accordance with MGB Home Care's policies and standards of practice.
- Utilize thorough and timely electronic documentation and interdisciplinary communications to ensure seamless care delivery.
- Collaborate with healthcare providers to develop and implement individualized care plans that meet patients' unique needs.
- Communicate effectively with patients, families, and healthcare teams to ensure high-quality customer service and financial awareness.
- Graduate of an approved School of Nursing.
- Current Massachusetts license as Registered Nurse.
- Minimum of one year previous nursing experience in an acute care setting, with prior home care experience preferred.
- Ability to work with various computer software and knowledge of hand-held patient care computer devices preferred.
- Strong interpersonal and customer service skills, with the ability to work with a diverse population.
- Ability to communicate effectively in writing, verbally, and electronically.
- Ability to work as a member of an interdisciplinary team of healthcare providers.
- Ability to organize and prioritize work, and adapt to changing situations.
- Ability to work independently, be self-directed, and adapt to unpredictable circumstances.
