Patient Care Coordinator RN

We are seeking a highly skilled Registered Nurse to join our team as a Patient Care Coordinator. In this role, you will be responsible for coordinating patient care, working closely with the interdisciplinary team, and ensuring the highest level of quality service.
Key Responsibilities- Perform daily rounds on all patients to ensure quality customer service and address any requirements not met.
- Assign or delegate nursing interventions to other staff members based on their skills and competence.
- Begin discharge planning, education, and counseling to optimize patient understanding.
- Communicate with referring facilities and agencies regarding patient care.
- Monitor patient outcome measurements and identify areas for improvement.
- Collaborate with the team, referral sources, and family members to obtain a holistic view of the patient.
- Determine post-hospital needs and work with physicians and the multidisciplinary team to address them.
- Communicate with staff and Nursing Administration regarding bed availability and staffing needs.
- Perform pain assessment and take appropriate action.
- Registered Nurse licensure in North Dakota or a Compact State Agreement.
- Basic Life Support (BLS) certification within 90 days of start date.
- Additional specialty certification/training as required by the work area.
- A minimum of 2 years of related experience.
This position requires frequent standing, walking, and stooping/bending. The ability to lift up to 50 pounds is also required.
