Job Summary

We are seeking a highly skilled Registered Nurse to join our team as a Patient Care Coordinator. In this role, you will be responsible for coordinating patient care, working closely with the interdisciplinary team, and ensuring the highest level of quality service.

Key Responsibilities
  • Perform daily rounds on all patients to ensure quality customer service and address any requirements not met.
  • Assign or delegate nursing interventions to other staff members based on their skills and competence.
  • Begin discharge planning, education, and counseling to optimize patient understanding.
  • Communicate with referring facilities and agencies regarding patient care.
  • Monitor patient outcome measurements and identify areas for improvement.
  • Collaborate with the team, referral sources, and family members to obtain a holistic view of the patient.
  • Determine post-hospital needs and work with physicians and the multidisciplinary team to address them.
  • Communicate with staff and Nursing Administration regarding bed availability and staffing needs.
  • Perform pain assessment and take appropriate action.
Requirements
  • Registered Nurse licensure in North Dakota or a Compact State Agreement.
  • Basic Life Support (BLS) certification within 90 days of start date.
  • Additional specialty certification/training as required by the work area.
  • A minimum of 2 years of related experience.
Working Conditions

This position requires frequent standing, walking, and stooping/bending. The ability to lift up to 50 pounds is also required.
