Mendocino Coast Clinics is seeking a highly skilled Substance Abuse Disorder Case Manager to join our team. As a key member of our Integrated Behavioral Health team, you will play a critical role in providing comprehensive care to our patients struggling with substance abuse.

Key Responsibilities:

  • Provide case management services to Suboxone patients, including coordination of care with Suboxone Providers, Integrated Behavioral Health Providers, and other community agencies.
  • Act as a liaison between patients, providers, and community agencies to ensure seamless care coordination.
  • Provide direct services to patients as appropriate, including counseling and support.
  • Collaborate with the MAT team to ensure comprehensive care for patients.
  • Develop and maintain relationships with community agencies and providers to enhance care coordination and access to services.

Requirements:

  • College degree and certification in substance abuse counseling preferred.
  • Knowledge of recovery support groups and process.
  • Experience in SUD, crisis management, health and human services, or case management strongly preferred.
  • Understanding and familiarity with medical terminology preferred.
  • Bi-lingual preferred.
