Position Overview:
The Registered Nurse Case Coordinator is responsible for overseeing the care and services provided to patient populations from the point of admission through to discharge.
This role involves collaborating with interdisciplinary teams to develop a comprehensive case management strategy, ensuring effective care coordination and optimal utilization of healthcare resources to meet clinical and financial objectives.
Key responsibilities include partnering with the healthcare team to address all aspects of the patient's needs—clinical, psychosocial, and financial—during the transition of the care plan, while managing the patient's timely progression of care and safe transition to the next appropriate level of care.
Key Responsibilities:
Conducts ongoing chart reviews to identify actual or potential issues related to service delivery, patient outcomes, satisfaction, compliance, cost, and reimbursement.
Monitors patient and family satisfaction, addressing inquiries and concerns from patients, family members, and payers regarding care.
Makes appropriate referrals to Social Workers and identifies suitable hospice candidates, communicating options with physicians.
Engages with physicians and multidisciplinary teams regularly to assess the patient's status and the appropriateness of medical care, including admission, length of stay, transfer, and discharge.
May collect and analyze data to identify quality trends or potential risks to patients and staff. Collaborates with multidisciplinary teams and committees to enhance quality outcomes.Competencies:

Education and Certification Requirements:
Accredited Program:
Nursing (Required)
BLS Certification (American Heart Association) - Required upon hire.
Additional Job Information:
This role requires critical thinking skills, effective communication, decisive judgment, and the ability to foster positive relationships. The incumbent must be capable of leading others and taking appropriate action when necessary.
Must provide education and resources relevant to the effective progression of care, utilization of services, appropriate level of care, and safe patient transition to the patient/family and healthcare team.
Required Work Experience:
A minimum of one (1) year of experience in a hospital-based Case Management practice or five (5) years in Healthcare.
