Job Summary:

Trinity Health is seeking a skilled Clinical Relief Pool Community Services Specialist to join our team. As a member of our Clinical Relief Pool, you will provide temporary or substitute staffing to support the delivery of high-quality patient care.

Key Responsibilities:

Provide immediate safety and supportive counseling in a recovery environment for persons served.
Conduct case management, skills building, supportive counseling, drug and alcohol services, medication delivery, medication observation, and linkage to appointments and community resources.
Collaborate with all team members to ensure continuity of care.
Maintain accurate and thorough documentation daily.

Requirements:

Master's degree in psychology, Social Work, or Human Services plus 1 year of paid relevant experience.
OR Bachelor's Degree in Human Services and 3 years of paid relevant experience.
Act 33/34/73 clearances.
Pre-Employment Physical/TB/Hep B.
Pre-employment Drug Screen.
Valid driver's license.
Ability to drive an organization vehicle/van.

Work Schedule:

This is a Non-Exempt (hourly) position. Shifts are dependent upon program needs; but fall within Monday through Friday 8:00 am until 4:00 pm with flexibility to meet consumers' needs. Non-traditional hours (evenings/weekends/holidays) may be required. Must work a minimum of 5 shifts per month.
