As a LPN Homecare Specialist, you will be responsible for assessing, planning, implementing, and evaluating patient care, ensuring the best possible outcomes for our patients.

Shift Options:
  • 7:00PM - 7:00AM (FT Rotation x2)
  • 8:00PM - 8:00AM (PT/PRN)
  • 11:00PM - 7:00AM (Sun, Wed, Thu)
  • 6:00PM- 6:00AM (FT Rotation)
  • 6:30PM - 6:30AM (FT Rotation)
  • 11:00PM - 7:00AM (Saturdays)
  • 8:30PM- 7:00AM (Tuesday-Friday)

Responsibilities:
  • Utilize the nursing process to assess, plan, implement, and evaluate patient care.
  • Provide skilled nursing care/services in accordance with prescribed orders.
  • Assess signs and symptoms indicating physiologic and psychosocial changes in the patients condition.
  • Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.
Qualifications:
  • Currently licensed as an LPN/LVN in the state in which the LPN/LVN will practice.
  • Current TB or Chest X-Ray.
  • Current BLS card.
