Overview
Amedisys is a leading provider of home health, hospice and high-acuity care, dedicated to helping patients and families navigate the complex healthcare system. We are seeking a highly skilled and compassionate registered nurse (RN) to join our team of professionals committed to improving the lives of those they serve.
Responsibilities
- Coordinates and oversees all aspects of patients' home care, ensuring timely and accurate documentation.
- Collaborates with patient care teams to develop and implement care plans that meet the unique needs of each patient.
- Provides education to families on patient condition, needs, and care, promoting a supportive and inclusive environment.
- Performs head-to-toe physical assessments, checks vital signs, and manages medications to ensure optimal patient outcomes.
- Administers and maintains catheters, gives injections, IV therapy, or tube feedings as needed, and completes documentation in a timely and accurate manner.
- Supervises LPNs and HHAs, providing guidance and support to ensure high-quality patient care.
- Regularly communicates patient progress to the clinical manager, physician, and care team, ensuring seamless coordination of care.
Qualifications
- One year of clinical experience as a registered nurse (RN), with a minimum of six months home health experience.
- Current RN license, specific to the state(s) assigned to work, and current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
- Able to assess patient status and identify requirements relative to age-specific needs, and must be familiar with general use and functions of the computer.
