Job Summary

We are seeking a skilled Case Manager to join our team at South Middlesex Opportu. The successful candidate will be responsible for providing case management services to clients transitioning from emergency shelter into short-term housing.

Key Responsibilities
  • Case Management Services: Provide initial and ongoing assessment, regular follow-up, and referrals/linkages to community-based services for assigned clients.
  • Service Planning: Establish a service plan with each individual, including goals and objectives, and ensure adherence to the plan.
  • Community Resource Referrals: Provide related information and referrals to community-based resources, including Fuel Assistance, Food Stamps, food pantries, and other services that can assist clients in maintaining sustainable housing.
  • Service Coordination: Coordinate all services required by the service plan, including housing and employment search and stability.
  • Client Support: Establish a respectful relationship with clients, schedule regular home visits to monitor the service plan, and maintain weekly contact to re-assess, evaluate, and document adherence to the service plan.
  • Advocacy: Advocate in Housing Court and assist clients in establishing positive relationships with landlords.
  • Documentation: Maintain up-to-date case notes, telephone contact logs, and referrals to community-based services, and enter all client data into the required database(s) on a daily basis.
  • Team Collaboration: Participate as a member of the Case Management team, attend all team meetings, and conduct groups and workshops related to clients' needs as appropriate.
  • Transportation Assistance: Assist clients in arranging appointments and transportation.
  • Confidentiality: Maintain confidentiality of client, employee, and agency information in accordance with federal and state laws and funder requirements.
  • Compliance: Ensure compliance with program/department, agency, and/or funder requirements, as well as SMOC policies and procedures.
Requirements
  • Education: Bachelor's Degree preferred, or a minimum of three years of experience in Human Services or a related field.
  • Experience: Minimum one year of assessment, advocacy, and case management skills.
  • Skills: Sensitivity to low-income people of diverse backgrounds, excellent listening skills, ability to work independently, good written and verbal communication skills, and bi-lingual preferred.
  • Physical Requirements: Ability to drive a vehicle and ascend/descend stairs in residences.
