Key Responsibilities- Monitor and manage medication therapy regimens for patients with hematological disorders
- Conduct daily in-depth patient profile reviews to assess, monitor, and recommend appropriate drug therapy
- Identify and intervene on adverse drug reactions, medication variances, and clinical interventions beyond institutional protocols
- Collaborate with healthcare teams to ensure safe and effective medication use
- Provide education and training to pharmacy students, technicians, and pharmacists on medication management and clinical practices
- Graduate of an accredited school of pharmacy
- ASHP-accredited Post Graduate Year 1 and 2 Residency or Fellowship or equivalent experience in the specialty area
- Active Georgia Pharmacist license or eligibility for licensure with attempt to register for 60 days of hire
- Basic Life Support (BLS) Certification in accordance with the American Heart Association required
- Advanced Cardiac Life Support is required within 1 year for all positions with Code Blue coverage responsibility
