Position: Center Medical Director

Location: Springfield, IL

Compensation: $250,000-$260,000

Position Overview:

The Center Medical Director at Oak Street Health is responsible for guiding the clinical team in delivering exceptional patient care while fostering a supportive work environment. This role involves practicing primary care in our clinics, dedicating 80% of time to clinical duties and 20% to administrative responsibilities. The CMD will play a vital role in leadership, exemplifying best practices in primary care, mentoring providers, and engaging in key strategic initiatives across the organization.

Key Responsibilities:

The Center Medical Director serves as the medical authority for their respective center, with the following key duties:

Support of Organizational Goals
  • Collaborate closely with the Practice Manager and Outreach Manager/Director to meet organizational objectives:
    • Delivering the Best Care Anywhere

  • Facilitate the execution of strategic initiatives.
  • Follow guidance from regional leadership to implement regional priorities and achieve objectives.
  • Communicate organizational updates to care team members and elevate team concerns to the broader organization.
Provider Oversight
  • Assist in the onboarding process for new providers at the center.
  • Demonstrate exemplary medical care and successful metric implementation.
  • Ensure effective execution of daily huddles, patient care planning, and clinical documentation workflows.
  • Guarantee that care provided is evidence-based and focused on keeping patients healthy.
  • Educate and implement Oak Street's clinical guidelines at both individual and center levels.
  • Support care teams in delivering high-quality preventative care.
  • Provide ongoing education for providers on the value-based care model.
  • Identify and address root causes when performance metrics are not met.
Leadership and Development
  • Foster a culture of growth and development among providers and care teams.
  • Actively listen to feedback and address concerns with constructive solutions.
  • Conduct performance evaluations and assist providers in setting and achieving their goals.
Workplace Culture
  • Recognize and celebrate the achievements of care team members while identifying areas for improvement.
  • Implement best practices for team collaboration.
  • Promote a positive workplace culture that encourages joy in practice.
  • Perform additional duties as required.
Ideal Candidate Profile:

Successful candidates will possess:
  • M.D., D.O., or A.P.N or P.A. qualifications.
  • Board certification in Internal Medicine or Family Medicine for M.D. or D.O.; Board certification in Family, Adult, or Gerontology for A.P.N.
  • Experience in clinical leadership roles, guiding and mentoring providers.
  • Familiarity with metrics-driven healthcare and quality improvement initiatives.
  • A passion for healthcare and a desire to contribute to a rapidly growing organization.
  • A strong sense of leadership and the ability to impact a dynamic team.
  • A commitment to serving older adults and low-income patients.
  • Adaptability and problem-solving skills in ambiguous situations.
  • Composure in challenging circumstances, guiding teams towards effective solutions.
  • 5+ years of outpatient practice experience preferred.
  • Fellowship training in Geriatrics or other advanced degrees is a plus but not mandatory.
  • Authorization to work in the U.S.
