Key Responsibilities- Advocacy and Leadership: Serve as a key player in delivering exceptional care, leading a team of clinical professionals, and collaborating with them to enhance the overall quality of life for our HCBS residents.
- Clinical Oversight: Oversee clinical team members in the field, ensuring adherence to Best Practices established by the Board of Nursing and CAHC Accreditation Standards.
- Communication and Collaboration: Maintain open communication with other Team Members, Clients, and Family, providing input regarding placing certified home health aides (CHHAs) appropriately with clients.
- Supervision and Evaluation: Supervise CHHAs in the greater community, making monthly follow-up calls/visits to ensure adherence of Care Plan, and evaluating the effectiveness of care provided by CHHAs with timely completion of 60-day skills assessments during on-site visits.
- Education and Licensure: New Jersey Registered Nurse (RN) License, BSN preferred.
- Certifications and Experience: BLS Certification, New Jersey Driver's License, and an automobile in good working condition with appropriate insurance coverage in accordance with New Jersey laws. Minimum of one-year supervisory experience in a health care setting, preferably home care, hospice, or long-term care.
- Skills and Abilities: Exceptional communication and interpersonal skills, robust critical thinking and problem-solving abilities, capable of working independently and collaboratively in a multidisciplinary team, and proficient in electronic medical record systems and relevant software applications.
