We are seeking a skilled healthcare professional to join our team as a Nurse Case Manager in Lodi, WI.

Dove Healthcare is a leading provider of innovative and compassionate care in Wisconsin, with 11 skilled nursing and rehabilitation centers, six assisted living residences, and three independent living complexes.

About the Role:

  • As a Nurse Case Manager, you will be responsible for coordinating patient care services, supervising daily activities, and ensuring residents receive high-quality care.
  • You will work closely with healthcare teams to develop personalized care plans, implement treatment strategies, and evaluate patient progress.
  • This role requires strong communication and leadership skills, as well as the ability to work effectively in a fast-paced environment.

Requirements:

  • A current licensure in the State of Wisconsin as a registered nurse (RN)
  • A degree from an accredited school of nursing
  • Previous nursing experience, preferably in a case management or leadership role
