The Assistant Program and Outreach Director plays a vital role in the execution of resident-focused activities that embody the Phoenix Senior Living philosophy. This position emphasizes wellness across all facets of the lives of the seniors we serve, ensuring that emotional, physical, social, intellectual, and spiritual dimensions are woven into the services provided.

Key Responsibilities:

  • Review the Resident Profile and addendum for each new resident.
  • Facilitate a diverse range of personalized activities that promote wellness and are accessible throughout the day and evening, collaborating with resident care associates.
  • Recognize changes in residents' conditions and take appropriate actions.
  • Assist residents in navigating to and from activity rooms as needed.
  • In memory care, support residents in participating in Life Skills and other enriching activities.
  • Maintain accurate documentation in the communication log regarding residents' physical and behavioral changes.
  • Ensure activity areas are organized and well-maintained.
  • Oversee the care of any animals or plants involved in the activities program, including record-keeping.
  • Help manage inventory for activity supplies, games, programs, and crafts.

Qualifications:

  • Education: High School Diploma required; experience in a senior living environment is preferred.
  • Ability to lead activities and inspire volunteers and colleagues to engage in social events and scheduled activities.
  • Familiarity with the Assisted Living sector and experience in Dementia care is advantageous.
