Key Responsibilities:- Evaluate and address the needs of individuals in their homes, ensuring they can live safely and productively.
- Develop and implement personalized treatment plans to address areas such as home safety, medication management, wheelchair seating and positioning, pain management, falls prevention, and management of chronic diseases.
- Collaborate with the Case Manager to provide comprehensive care and support to patients and their families.
- Conduct home visits to assess patients' needs and provide education on safe and effective use of equipment and techniques.
- Flexible work schedules and autonomy to create your own schedule and hours.
- Targeted territories with smaller coverage areas, allowing for a better work-life balance.

- Current unrestricted CA Occupational Therapist License.

- Experience in home health care and working with high acuity patients is highly preferred.
