We are seeking a skilled RN - Case Manager to join our team at Sacred Heart Hospital on the Gulf. As a Case Manager, you will play a critical role in coordinating patient care and ensuring seamless transitions between healthcare services.
Key Responsibilities- Develop and implement comprehensive care plans that meet the unique needs of each patient
- Collaborate with interdisciplinary teams to ensure effective communication and coordination of care
- Monitor patient progress and make adjustments to care plans as needed
- Advocate for patients and their families to ensure access to necessary resources and services
- Maintain accurate and up-to-date patient records and reports
- Licensure as a Registered Nurse in the state of Florida
- Current BLS certification
- Experience in case management or a related field
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
