Home Health Weekend Registered Nurse RN

The Registered Nurse (RN) will be responsible for providing skilled nursing care to patients in their homes on weekends. The RN will be on call from Friday at 4:30 pm until Monday at 8:00 am (Eastern) and will be responsible for covering any as-needed visits that arise, as well as scheduled weekend visits, including admission visits.
Key Responsibilities:- Provide skilled nursing treatments as ordered by the client's doctor
- Complete evaluation tasks, including vital signs and medication reviews
- Coordinate with physicians, therapists, and other individuals in the client's care plan
- Assess and chart observations of the client's condition at each visit
- Maintain complete and accurate documentation of nursing care
- Work collaboratively with all members of the healthcare team
- Provide education to clients and families on proper home health care procedures and strategies
- Record patients' medical information and vital signs
- Monitor, record, and report symptoms or changes in patient's conditions
- Must be available on weekends from Friday 4:30 pm until Monday 8:00 am (Eastern)
- Associates degree required; Graduate of an accredited state-approved, Bachelor's Degree from a school of nursing preferred
- Current and valid California Registered Nurse License
- Prior nursing experience, specifically in home health (preferred)
- Current CPR certification (required)
- Driver's License
- Car Insurance
- Car Registration
- Strong interpersonal skills that build personal rapport and establish trust with the client
- Computer literate
- Strong organizational skills
- Excellent written and communication skills
- Ability to meet physical demands of the job (ex: lifting 50 pounds for transfers)
