As a leader of the healthcare team, this Home Health RN will provide professional nursing care, utilizing the nursing process in accordance with established standards of care, policies, and procedures. The ideal candidate will demonstrate performance consistent with the Medical Center Vision, Mission, and Strategic Plan of the organization.
Responsibilities include:
- Providing patient care that meets standards of practice
- Developing and implementing individualized plans of care
- Collaborating with the healthcare team to ensure effective patient care
- Communicating effectively with patients, families, and healthcare team members
- Participating in quality improvement initiatives
Requirements:
- Minimum one year recent experience in a licensed Home Health, Palliative Care and/or Hospice agency as an RN/PHN
- Registered Nurse License (California)
- Driver's License (California)
- Basic Life Support
Additional Requirements:
- Clean Driving Record for the last two years
- Proof of automobile insurance as required by law
Notes:
- Required to work every other weekend
- May support Home Health, HH with Palliative Pathways and AMCAH program
