We are seeking a compassionate and skilled Nurse Practitioner to join our team at Abode Care Partners, where we provide personalized medical care to seniors and individuals with complex needs in the comfort of their own homes.

Job Summary: As an In-Home Medical Provider for Complex Needs, you will evaluate patients, manage chronic and urgent health issues, and collaborate with healthcare professionals to maintain comprehensive care plans. - Collaborate with healthcare professionals to maintain comprehensive care plans
- Provide personalized medical care to seniors and individuals with complex needs
- Work in various residential settings, including skilled nursing facilities, assisted living, independent living, group homes, and private residences
Requirements:
- Active state Nurse Practitioners license
- Current Nurse Practitioner National Board Certification
- Active state DEA licensure
- Master's degree in Nursing (MSN)
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
- Strong interpersonal communication skills - verbal and written
- Excellent time management skills with accountability for practitioner's daily workflow
- Proficiency in medical instruments and equipment
- Strong knowledge base of HIPAA and OSHA regulations
- Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications
- Licensed driver with valid automobile insurance
