We're seeking a skilled House Calls Nurse Practitioner (FNP/AGNP - APRN) to join our team and provide personalized care to patients in their homes, assisted living facilities, or independent living communities.

Key Responsibilities:

  • Evaluate patient medical needs and develop comprehensive care plans
  • Manage chronic and urgent health issues, including minor injuries and episodic illnesses
  • Collaborate with healthcare professionals to maintain interdisciplinary care and ensure seamless communication
  • Provide referrals to other healthcare practitioners when necessary and ensure accurate documentation of patient records

Qualifications:

  • Active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • Master's degree in Nursing (MSN)
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
