The Complex Care Social Worker will utilize a structured escalation protocol to identify patients with barriers to discharge, collaborating with the interdisciplinary team to develop and implement a plan of care.

Key responsibilities include:

  • Conducting thorough assessments to identify patient needs and develop individualized plans
  • Collaborating with the care team to ensure timely and effective discharge
  • Monitoring high-cost and difficult-to-place patients to identify early intervention opportunities
  • Providing leadership and creativity to enhance the effectiveness of care management

Requirements:

  • Master's degree in social work or related field
  • Current/Valid license in the State of Maryland; LCSW-C/LCPC required
  • Minimum of 3 years of experience; 5+ years of experience preferred
