Key Responsibilities- Terminal Care Management: Develop and implement effective care plans for patients with terminal illnesses, ensuring their comfort and dignity are maintained.
- Pain and Symptom Management: Utilize expertise in pain and symptom management to provide high-quality care to patients and their families.
- Staff Supervision: Supervise and mentor a team of staff members, providing guidance and support to ensure excellent patient care.
- Documentation and Compliance: Ensure accurate and timely documentation of patient care, adhering to federal, state, and local laws and regulations governing hospice operations.
- Service Coordination and Case Management: Oversee the coordination of patient care services, including referrals to community resources and other healthcare providers.
- Education: Graduate of an approved school of professional nursing, currently licensed as a Registered Nurse in the state of employment.
- Experience: Minimum of two years of nursing experience within the past 36 months, with at least one year of experience in a hospice setting.
- Skills: Strong communication and leadership skills, with the ability to execute an effective and supportive approach in response to patient and family needs.
