Position Details:

Key Responsibilities:

  • Ensure accurate documentation of nutritional care through assessments and progress notes.
  • Provide dietary counseling to patients to promote health and wellness.
  • Evaluate the dietary and health requirements of patients.
  • Guide patients on nutrition and food choices.
  • Create individualized meal plans and monitor their effectiveness.
  • Participate in team, unit, and nutrition meetings to enhance care delivery.
  • Observe patients during meal times to assess their dietary habits.
  • Assist in managing food production processes.
  • Engage in professional development through meetings and seminars.
  • Perform additional duties as required.

Qualifications:

  • Strong ability to connect with patients.
  • Empathetic and supportive attitude.
  • Knowledge of medical terminology.
  • Current CPR certification (American Heart Association).
  • Valid credentials in the state of CA.
