We are seeking a dedicated and caring professional to join our Housing team at Old Colony Elder Services. As a Housing Specialist, you will play a vital role in ensuring the well-being and independence of our residents.

Key Responsibilities:
  • Conduct needs assessments to identify residents' requirements and connect them with community programs and services.
  • Lead the Multi-disciplinary Assessment Team and facilitate care conferences with residents, family members, and other stakeholders.
  • Plan and coordinate social, educational, and recreational programs for residents.
  • Prepare and distribute a newsletter to keep residents informed of upcoming activities.
Requirements:
  • Passion for helping others and a desire to make a positive impact.
  • Adaptability to respond to changing consumer and program needs.
  • At least two years of experience in the human services field, preferably with the elderly population.
  • Bachelor's degree in Social Work or a related field is preferred.
