Responsibilities
  • Assess patient psychosocial and continuum of care needs
  • Develop and implement interventions to address patient needs, including family support, crisis management, and referrals to community resources
  • Collaborate with care teams, patients, and families to ensure seamless transitions to least restrictive care environments
  • Apply expertise in social work to deliver high-quality care and adhere to Norton Healthcare policies
Qualifications
  • Master's degree in Social Work from an accredited program
  • One year of social work experience or internship
  • Kentucky Certified Social Worker (CSW) or Indiana Licensed Social Worker (LSW) certification
  • Desired: one year of experience in a healthcare setting and Licensed Clinical Social Worker or Licensed Social Worker certification
