Key Responsibilities
  • Act as a liaison between patients, families, and physicians to ensure seamless communication and care coordination.
  • Provide updates on patient condition to families and physicians, ensuring they are informed and engaged in the care process.
  • Ensure all in-home medical equipment is in working order, promoting patient safety and comfort.
  • Document all details of patient's condition, progression, and regression activity, maintaining accurate and comprehensive records.
  • Oversee required physical therapy or rehabilitation exercises, promoting patient recovery and independence.
  • Contact emergency services when patients require hospitalization, ensuring timely and effective care.
  • Possess knowledge of Home Care reimbursement processes and OASIS requirements, ensuring compliance and efficient care delivery.
  • Have reliable transportation for home visits, ensuring timely and convenient care.
  • Be able to walk up/down six flights of stairs as needed, demonstrating physical ability and flexibility.
Requirements
  • Minimum of one year hospital Med Surg experience, with a strong foundation in patient care and nursing practices.
  • One year of Home Health experience or Certified Home Care (CHHA) experience, with a proven track record of delivering high-quality care.
  • NYS RN License and Registration in good standing, ensuring compliance with regulatory requirements.
  • Bachelor's degree in Nursing, with a strong academic foundation and commitment to lifelong learning.
  • Infection Control certificate, demonstrating expertise in infection prevention and control.
  • Valid Driver License, Registration, and Automobile Insurance, ensuring safe and reliable transportation.
  • Knowledge of a second language a plus, promoting effective communication and care coordination.
