Job Summary

The Registered Nurse (RN) at Neighborcare Health plays a vital role in optimizing the health status of patients across the lifespan. As a member of our interdisciplinary team, you will work in partnership and joint accountability to achieve our mission, guiding principles, and goals. Your primary focus will be on chronic condition management, patient education, emergent triage, and performing complex procedures. You will build relationships with patients through face-to-face and telephonic care, working in partnership with your colleagues to meet the needs of patients throughout Neighborcare Health. You will follow the Nursing Process to assess, plan, implement, and evaluate nursing care in a Patient Centered Medical Home.

Key Responsibilities:

Working in Partnership and Joint Accountability: Provide care to patients in the outpatient clinic setting, work as a member of the nurse team, and maintain joint accountability to the clinic team and medical program. You will support colleagues throughout the organization, either on-site or remotely as needed, and participate in Quality and Performance Improvement efforts to improve patient outcomes. You may serve as a preceptor for new staff and/or students who are assigned to Neighborcare as part of their professional education, and provide community outreach to people who are experiencing homelessness in collaboration with the clinic team and community partners.

Chronic Condition Management: Establish partnerships with patients and families in person and via telephone for positive outcomes in health status. You will identify patient goals for improving their health status and management of chronic physical and mental health conditions, assess patient understanding of their condition and readiness to change, and provide patients with education about the disease process. You will offer tools and skills needed to achieve self-management goals, monitor, track, and follow-up with patients to ensure care needs are met, and coordinate care with outside providers and resources. You will apply Neighborcare Health guidelines and standing orders and evidence-based clinical practices, and offer medication-assisted treatment (MAT) in collaboration with prescribing providers.

Patient Education: Provide patient education across the lifespan in person and over the phone, assessing barriers to learning and tailoring education to meet patients' individual learning styles and needs. You will educate patients about medications, abnormal laboratory results, and self-management of acute and chronic conditions, promote wellness and the importance of preventive care, and provide motivational interviewing and harm reduction counseling.

Triage Emergent Health Issues and Perform Select Clinical Procedures: Respond to emergent health issues in person and via telephone to assess, advise, and provide first aid and Basic Life Support as needed. You will perform complex procedures as appropriate for the RN scope of licensure and individual competency, administer select injections and medications, and work in partnership with the clinic team and community partners to provide care to patients in the outpatient clinic setting.

Requirements:

Completion of an accredited RN program
Valid Washington State Registered Nurse license
BLS certificate

Preferred Qualifications:

1+ years RN experience in a primary care, hospital, and/or home care setting or comparable social service setting
Board Certification in applicable specialty areas
BSN
