As a Senior Wound Care Specialist, you will play a critical role in our team's success, working closely with patients and healthcare professionals to provide comprehensive wound care.

Job Overview

This is a unique opportunity to work in a dynamic and supportive environment, where you will have the chance to make a real difference in people's lives. You will be responsible for providing expert wound care to patients in their homes, using your skills and knowledge to promote healing and prevent complications.

The ideal candidate will have a strong background in nursing, with experience in wound care and management. You will be required to travel to cases as assigned, working independently and managing your time effectively.

Responsibilities
  • Provide expert wound care to patients in their homes, using evidence-based practices and protocols.
  • Assess patients' wounds and develop individualized care plans to promote healing and prevent complications.
  • Collaborate with healthcare professionals to coordinate care and ensure seamless transitions between services.
  • Monitor patients' progress, reporting changes to clinical managers and making adjustments to care plans as needed.
  • Document observations, interventions, and evaluations in accordance with company policies and regulatory requirements.
Qualifications
  • A current license as a Registered Nurse in Vermont.
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
  • Wound Care Certified.
  • Prior home care experience strongly preferred, but not required.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
