Job Overview:
We're seeking a skilled Primary Care Nurse Practitioner to join our Riverside clinic team. As a key member of our interdisciplinary team, you'll provide high-quality patient care, working closely with physicians and other healthcare professionals to diagnose and treat patients with various health concerns. Your expertise will be invaluable in promoting evidence-based medicine and value-based care practices.
Your primary responsibilities will include:
- Obtaining detailed medical histories from patients and performing thorough physical examinations.
- Reviewing patients' medication lists and providing education on proper usage and potential side effects.
- Conducting home safety assessments and identifying psychosocial needs.
- Fulfilling documentation requirements for quality metrics and HCC measures.
- Collaborating with patients, families, and healthcare providers to ensure seamless care coordination.
- Maintaining accurate and up-to-date patient records, including medications and end-of-life preferences.
- Providing timely and effective communication to patients and their caregivers.
- Implementing universal precautions in all patient interactions.
- Prescribing and ordering procedures, treatments, and medications within scope of practice.
- Building strong relationships with patients and families, fostering trust and intimacy.
We offer a competitive compensation package that reflects your skills, experience, and qualifications. The estimated salary range for this role is $120,000 - $150,000 per year, depending on your background and performance. Additionally, you'll enjoy a comprehensive benefits program, including medical, dental, and vision coverage, as well as retirement savings options and paid time off.
