We are seeking a highly skilled and motivated Community Health Specialist to join our team at Med Center Health. The successful candidate will be responsible for providing medical nutrition therapy services and promoting community wellness programs.
Key Responsibilities- Program Planning and Delivery: Assist with planning and delivering community events, initiatives, and wellness programs, including CPR certification, MCH 10k Classic, MCH Health & Wellness Expo, and the annual Women's Conference.
- Marketing and Outreach: Market medical nutrition therapy services and other programs to providers in the area and surrounding counties, including in-person and virtual events, as well as social media and other marketing campaigns.
- Community Education: Provide education to the community at health fairs and local events, promoting community activities that work toward the objectives set forth in the Community Health Needs Assessment.
- Patient Care: Assess patients' nutritional status, initiate outpatient records, and provide nutritional teaching and counseling to patients and their families.
- Collaboration and Communication: Participate in internal and external committees and resource groups, providing clinical expertise and representation for Community Wellness programs.
- Education: Bachelor's degree in dietetics and a completed internship program required.
- Certifications/Licensure: Licensed in the state of Kentucky and Commission on Dietetic Registration (Academy of Nutrition and Dietetics) required.
- Experience: Two years of clinical experience in an acute hospital setting preferred, with previous experience providing outpatient medical nutrition therapy counseling also preferred.
