Activities Leader Job Description

Southbrook Healthcare Center, a member of the CommuniCare Family of Companies, is seeking a skilled Activities Leader to join our team. As an Activities Leader, you will be responsible for planning, developing, and implementing a variety of activities to promote the physical, emotional, and social well-being of our residents.

Key Responsibilities:
  • Lead a range of activities, including one-to-one and group activities, outings, and community events.
  • Provide supplies and equipment for residents to participate in individual activities.
  • Assist with planning, developing, and evaluating diversified, resident-centered activities.
  • Coordinate holiday and special events, religious services, and community involvement.
  • Decorate for holidays and special events.
  • Escort residents to and from activities as needed.
  • Provide one-on-one time with residents to follow up on behavioral episodes and interventions.
  • Attend community outings with residents.
  • Arrange for specialized services, such as library and talking book services.
  • Post activity schedules, posters, and announcements, and maintain bulletin boards.
  • Communicate and coordinate plans for assigned activities with the activity coordinator/activity director and other departments.
  • Involves residents in planning, developing, and evaluating the activities program.
  • Advocate for residents regarding their needs, interests, preferences, and rights.
  • Participate in resident council meetings, family council meetings, and resident/family community support groups.
  • May contact families/significant others for resident/family activities.
  • Maintain up-to-date activity documentation on individual residents.
  • Assist in monitoring programming in the center, outside, and in the community.
  • Assist residents with ADL needs, such as transfer, toileting, and feeding.
  • Supervise assigned volunteers.
  • Participate in fundraising events.
  • Promote positive relationships with individuals, businesses, and organizations in the community through outings and activities provided by volunteers.
  • Perform clerical needs of the department and other job-related tasks.
  • Attend and participate in scheduled training, education classes, and orientation programs to maintain and enhance quality of care.
  • Perform other related activities as assigned or requested.
Qualifications:
  • High school diploma or GED.
  • Prior work/life experiences, preferably in a healthcare setting.
  • Prior experience preferably with related software applications.
  • Ability to make independent decisions when circumstances warrant such action.
  • Ability to deal tactfully with team members, other staff, residents, family members, visitors, government agencies, and the general public.
  • Ability to plan, organize, and conduct a variety of activities.
  • Willingness to seek out new methods and principles and incorporate them into existing activities practices.
  • Basic computer literacy and skills.
  • Active state driver's license.
