The Hospice Admissions Coordinator is a key role at FC Compassus LLC, requiring strong communication and organizational skills. As a Hospice Admissions Coordinator, you will be responsible for processing patient referrals, coordinating admissions, and entering patient data in accordance with hospice requirements.
Key Responsibilities- Attend and participate in staff meetings, educational programs, and community events to stay updated on industry developments and best practices.
- Communicate identified needs and potential solutions to your supervisor, ensuring seamless coordination and efficient workflow.
- Process referrals from physicians, patients, families, and community members, verifying that all necessary information is complete and accurate.
- Contact physicians to secure admission orders and confirm patient eligibility for our services.
- Contact insurance companies to obtain pre-certification authorization, ensuring timely and smooth payment processing.
- Schedule appointments with patients and their families for informed consents and RN assessment visits, providing clear explanations of our services and benefits.
- Inform relevant team members of appointment times, promoting effective collaboration and teamwork.
- Complete the admission process according to agency/corporate policy, ensuring compliance with regulatory requirements and maintaining high-quality standards.
- Notify assigned interdisciplinary teams and ancillary providers of patient admissions, facilitating seamless care coordination.
- Enter admission data into our third-party system, accurately capturing essential information for future reference.
- Ensure patient charts are complete, all documentation is signed and dated, and charts are transferred to Team Coordinators in a timely manner.
- Market referral sources, promoting our services to long-term care staff, hospital staff, and physicians.
- Provide backup support for the Hospice Care Consultant as needed, demonstrating flexibility and adaptability.
To succeed in this role, you will require:
Requirements- A High school diploma or equivalent required; a Bachelor's degree preferred.
- Two to three years of related experience and/or training strongly preferred.
- Experience communicating with physicians, nurses, and other healthcare professionals highly preferred.
- Experience in insurance and Medicare/Medicaid billing and reimbursement preferred.
This role necessitates regular standing, walking, and manual dexterity, along with the ability to lift and move objects up to 25 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
$55,000 - $65,000 per year (estimated salary range based on national averages and industry standards).
