Nurse Practitioner

We are seeking a skilled Nurse Practitioner to join our team at Haven Hospice, a leading provider of high-quality care in a warm and serene environment. As a Nurse Practitioner, you will play a vital role in supporting patients and their families by providing compassionate and expert care.
Responsibilities- Provide medical care and palliative services to patients with terminal illnesses, in compliance with state Nurse Practice Act and under the supervision of a collaborating physician.
- Conduct thorough history and physical examinations to evaluate patients' medical conditions and health histories.
- Complete hospice face-to-face visit requirements to assess eligibility for certification and recertification.
- Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification.
- Maintain accurate clinical notes and communicate effectively with physicians and interdisciplinary teams.
- Ensure compliance with state and federal regulations and additional responsibilities as assigned.
- Graduate from an accredited School of Nursing and accredited Nurse Practitioner program.
- Current DEA license, if applicable to state regulations.
- Current nursing license in the state of practice.
- Current CPR certification.
- Master's degree with a minimum of one year NP experience.
- One year hospice or palliative care experience preferred.
- Accreditation from an approved certifying body for advanced practice nursing, as required by state of practice.
- Certification in a specialist area, such as Hospice and Palliative Nursing (CHPN) or Geriatrics, preferred.
