Job Summary

Professional Case Management is seeking a skilled Physical Therapy Assistant to provide high-quality care to our home care clients. As a Physical Therapy Assistant, you will work under the supervision of a qualified Physical Therapist to deliver physical therapy treatments and education to clients in their homes.

Key Responsibilities
  • Provide physical therapy services and education/training to clients in their homes following the physician's Plan of Care and the specific functions identified in the therapy care plan.
  • Complete visits in a timely manner and in accordance with Agency policy.
  • Document all services provided, observations, client progress, and recommendations on designated forms and submits to the agency within seven (7) days of providing service or per Agency policy.
  • Report changes/concerns or schedule changes to the therapist and/or the nursing supervisor on the day of the observed change.
  • Attend staff meetings and continuing education programs.
  • Participate in the Agency quality improvement activities pertinent to role of Physical Therapy Assistant.
  • Demonstrate knowledge of safety and infection control practices by compliance with Agency policies and procedures.
  • Demonstrate safe practice in the use of equipment.
  • Protect written confidential documents in a manner that prevents unauthorized access.
  • Adhere to all Agency policies about client privacy and security of information.
Requirements
  • Current license, registration or certification to practice as a Physical Therapy Assistant in the state(s) of employment or be qualified to take the physical therapy examination.
  • Graduate of an accredited physical therapy assistant program approved by the American Physical Therapy Association.
  • Minimum of six (6) months experience as a Physical Therapy Assistant.
