Job Summary:

Heartland Human Care Services is seeking a skilled Mental Health Clinical Practitioner to join our team. As a key member of our Home Study Post Release Services Program, you will provide specialized services to unaccompanied children, focusing on case management and counseling. Under the guidance of our Clinical Supervisor, you will develop and implement treatment plans, assess client needs, and provide individual and group therapy sessions. Your expertise will help shape the lives of these vulnerable individuals and contribute to our organization's mission and vision.

Responsibilities:
  • Provide clinical case management and counseling services to clients and their families.
  • Develop and implement treatment plans, assess client needs, and provide individual and group therapy sessions.
  • Establish and maintain therapeutic relationships with clients to facilitate the receipt of services.
  • Assess, plan, and supplement the client's level of functioning.
  • Maintain documentation of the client's treatment.
  • Provide individual counseling to address motivational and psychosocial problems of unaccompanied children.
  • Provide intense advocacy, crisis intervention, and problem-solving in a variety of areas for all clients.
  • Make referrals to other Organization units and external providers.
  • Develop, monitor, and enhance professional relationships with social service, health, and governmental agencies.
  • Participate in regular supervision meetings, team meetings, in-house training sessions, conferences, and program and operational planning.
Requirements:
  • Master's degree (M.A.) in clinical social work, counseling, or clinical psychology.
  • Minimum two (2) years of clinical practice experience in either a hospital or community setting providing group and/or individual therapy.
  • Eligible to become a Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC).
