Occupational Therapist

BAYADA Home Health Care seeks a skilled and compassionate Occupational Therapist to join our team in providing high-quality home health care to clients in Jim Thorpe, Pennsylvania.
Key Responsibilities:- Perform thorough evaluations of patients' functional status and develop individualized intervention plans to achieve favorable outcomes.
- Make home visits to clients in designated geographic territories and assess the home environment for hazards or barriers to independent living.
- Educate and instruct patients, family members, or patient representatives in rehabilitative care and activities necessary to promote health, safety, and independent living.
- Continually assess and revise the occupational therapy care plan, participating with nursing in the multidisciplinary care plan as appropriate.
- A current PA Occupational Therapist license.
- Prior home care experience preferred, but not required.
- A minimum of one year of recent work experience as an Occupational Therapist.
