About the Job

We are seeking an experienced Occupational Therapist to join our team in Jim Thorpe, Pennsylvania.

As a member of our multidisciplinary healthcare team, you will provide rehabilitative care and skilled nursing services to clients in their homes. This is a full-time or part-time opportunity to work as an integral member of a dynamic team dedicated to delivering exceptional patient care.

Responsibilities
  • Making home visits to clients in designated areas and performing thorough evaluations of their functional status.
  • Developing individualized intervention plans to achieve favorable outcomes for clients, assessing cognitive, psychological, and motor aspects of performance.
  • Evaluating the home environment for hazards or barriers to independent living, identifying equipment needs, and implementing environmental adaptations.
  • Continuously assessing and revising occupational therapy care plans, participating in multidisciplinary care planning, and educating patients and family members on necessary skills for improved health and safety.
Requirements
  • A current PA Occupational Therapist license and a minimum of one year of recent experience as an Occupational Therapist.
  • Prior home care experience is preferred but not required; strong interpersonal, organizational, and problem-solving skills are essential.
  • Ability to work independently, manage time effectively, and communicate with clients and colleagues.

We offer competitive compensation, with estimated salary range $85,000 - $110,000 per annum, depending on experience and qualifications.
