Job Summary:

Scout Exchange is seeking a skilled Medical Care Specialist to join our team in a Long-Term Care (LTC) setting. As a Medical Care Specialist, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to deliver exceptional patient outcomes.

Responsibilities:

Provide direct patient care to residents in a LTC setting, including assessing, planning, and implementing individualized care plans.
Collaborate with the healthcare team to develop and implement care plans that meet the unique needs of each resident.
Conduct thorough assessments of residents' physical and emotional needs, and develop plans to address these needs.
Administer medications and treatments as prescribed by physicians, and monitor residents' responses to these interventions.
Educate residents and their families on health-related topics, and provide support and guidance as needed.

Requirements:

1 year of experience working in a LTC setting, with a focus on geriatric care.
Strong assessment and planning skills, with the ability to develop and implement individualized care plans.
Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare team members.
Ability to work in a fast-paced environment, with a high level of autonomy and independence.

Working Conditions:

This position requires the ability to work in a LTC setting, with frequent exposure to residents with dementia, CHF, diabetes, CVA, COPD, depression, schizophrenia, and substance use disorders.
The ideal candidate will have reliable transportation and be able to work a flexible schedule, including evenings and weekends as needed.
