Care Coordinator Nurse

At Thresholds, we are committed to providing exceptional care to our clients. As a Care Coordinator Nurse, you will play a vital role in leading care coordination services and conducting physical health and preventative care screenings. You will work closely with our team to ensure the delivery of services that meet the needs of our clients through the coordination of care.

About the Role
- Monitors and ensures that health care information collected at intake and other intervals is distributed to staff, psychiatrist, and other medical personnel as necessary.
- Conducts preventive care screening and follow up, including BMI, weight, diabetes, and may conduct tobacco and alcohol screening, suicide and depression screening, and medication adherence screening and management.
- Utilizes service data to monitor and coordinate CCBHC consumers' healthcare integration with collaterals.

Requirements
- Licensed Registered Nurse required.
- Two (2) years experience working with persons diagnosed with mental illness preferred.
- Ability to synthesize and summarize information and make judgments regarding patient care.
- Must communicate effectively both verbally and in writing in order to provide information to patients and healthcare personnel.
- Demonstrated ability to function independently.
- Computer skills required; knowledge of database functions and maintenance necessary.
