Job Summary

We are seeking a compassionate and skilled LPN to join our team as an Individualized Care Specialist. In this role, you will provide high-quality nursing care to patients in their homes and facilities, working under the direction of an RN.

Key Responsibilities
  • Provide individualized care to patients with complex needs, including those who are actively declining.
  • Work collaboratively with the patient's care team to develop and implement personalized care plans.
  • Deliver superior clinical care to patients and their families, focusing on comfort, dignity, and quality of life.
  • Document services and care provided, utilizing computer skills to ensure accurate and timely documentation.
Requirements
  • Registered LPN with the State of Ohio, without any board actions.
  • One year of nursing experience preferred.
  • CPR certification.
  • Valid Driver's License with a safe driving record.
  • State Minimum Automobile Insurance Coverage.
