Job Summary

We are seeking a skilled Nurse Practitioner to join our team of healthcare professionals. As a key member of our geriatric care team, you will provide primary, urgent, and acute medical care to patients in their homes. Your expertise will be instrumental in delivering comprehensive care to our patients, addressing their unique needs and challenges.

Responsibilities
  • Provide primary care to patients in their homes, including routine check-ups, health screenings, and medication management.
  • Address urgent care needs, such as acute illnesses or injuries, and provide timely interventions to prevent complications.
  • Collaborate with our interdisciplinary team, including physicians, social workers, and care coordinators, to ensure seamless care coordination and patient outcomes.
  • Develop and implement individualized care plans, taking into account patients' medical histories, social determinants, and cultural backgrounds.
  • Communicate effectively with patients, families, and caregivers to ensure informed decision-making and patient engagement.
  • Stay up-to-date with the latest clinical guidelines, research, and best practices in geriatric care, and apply this knowledge to improve patient outcomes.
Requirements
  • Master's degree in Nursing or related field.
  • Valid nurse practitioner or physician assistant license in the state of operations.
  • 5+ years of progressive care experience, preferably in geriatrics or home health care.
  • BLS and ACLS certification, as well as an NPI number as an NP.
  • Valid driver's license, car insurance, and willingness to drive to patient homes.
