Job Summary

We are seeking a highly skilled and compassionate Ambulatory Care Navigator to join our team at the Greater Baltimore Medical Center (GBMC). As a key member of our care coordination team, you will play a vital role in ensuring the seamless delivery of patient care and promoting population health management.

Key Responsibilities
  • Manage a defined patient panel by providing linkages to community resources, assisting with scheduling appointments, and following up to ensure compliance with care plans.
  • Assess Social Determinants of Health (SDOH) for patients and connect them with community supports and services.
  • Collaborate with the care team to prevent unnecessary utilization and promote efficient care delivery.
  • Develop and implement care plans that address the unique needs of each patient.
  • Monitor population management data and reports to identify areas for improvement and develop targeted interventions.
Requirements
  • 3 years of medical office experience and experience navigating the healthcare system.
  • Knowledge of medical and insurance terminology and strong analytical skills.
  • Excellent communication and problem-solving skills with the ability to work effectively in a team environment.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
Preferred Qualifications
  • Bachelor's degree in a related field such as healthcare administration or social work.
  • Experience with electronic medical records (EMRs) and other healthcare technology systems.
  • Knowledge of population health management principles and care coordination best practices.
