Job Summary

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Oak Street Health Inc. as an Advanced Practice Provider. Key Responsibilities
  • Provide high-quality primary care to patients, including routine and acute visits, annual wellness visits, and health risk assessments.
  • Conduct thorough assessments, develop and implement individualized care plans, and educate patients on their health conditions, care plans, and treatments.
  • Collaborate with other healthcare professionals, specialists, and agencies to ensure comprehensive and coordinated care.
  • Participate in population health leadership, including the development and implementation of evidence-based care protocols and quality improvement initiatives.
  • Assist with phone triage, outreach, and other administrative tasks as needed.
Requirements
  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
  • National certification in at least one of the following specialties: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
  • Active, non-probationary state Nurse Practitioner license.
  • Active DEA license.
  • US Work Authorization.
Preferred Qualifications
  • Experience in primary care, internal medicine, geriatrics, or family medicine.
  • Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments.
  • Passion for teamwork and collaboration.
  • Desire to be part of an innovative model focused on empirically-guided population health.
