Key Responsibilities- Develop and implement individualized care plans for patients, taking into account their unique needs and goals.
- Conduct thorough assessments of patients' medical conditions and provide expert guidance to healthcare teams.
- Coordinate with healthcare providers, family members, and caregivers to ensure seamless care transitions and optimal patient outcomes.
- Monitor patients' progress, identify areas for improvement, and make adjustments to care plans as needed.
- Collaborate with interdisciplinary teams to develop and implement evidence-based care practices.
- Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle choices.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote best practices in patient care.
- Graduate of an accredited nursing program and licensed to practice in the state(s) where you will be working.
- Minimum one year of experience in a healthcare setting, preferably in a case management or clinical coordination role.
- Strong knowledge of healthcare regulations, policies, and procedures.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities.
- A competitive salary range of $75,000 - $80,000, depending on experience.
