About the Role

Vitas Healthcare is seeking a highly skilled Registered Nurse to join our team as a Palliative Care Nurse Specialist.

The ideal candidate will have a strong background in hospice care, excellent communication skills, and the ability to work effectively with patients, families, and healthcare teams.

Job Responsibilities
  • Provide palliative care services to patients and families in various settings, including home, hospital, and long-term care facilities.
  • Assess patient needs and develop individualized care plans in collaboration with the interdisciplinary team.
  • Administer medications, perform medical procedures, and provide education to patients and caregivers on symptom management and end-of-life care.
  • Maintain accurate and comprehensive documentation of patient interactions, assessments, and interventions.
  • Collaborate with healthcare providers, social workers, and other members of the interdisciplinary team to ensure seamless care coordination.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures related to palliative care.
Requirements
  • A minimum of 2 years of experience in hospice care, home health, or community health within the last 5 years.
  • Current licensure as a registered nurse in the state where the Vitas program is located.
  • Strong clinical skills, particularly in pain and symptom management, and experience with electronic medical records.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to travel to patient locations and participate in on-call rotations.
What We Offer

Vitas Healthcare offers a competitive salary range of $65,000 - $85,000 per year, based on experience, as well as opportunities for professional growth and development.
