Optum is seeking a skilled Nurse Practitioner or Physician Assistant to join our Senior Community Care team. As a field-based APC, you will provide high-quality care to our highest-risk health plan members in assisted living and long-term care facilities.
Key Responsibilities- Conduct comprehensive assessments and manage members through acute medical situations or exacerbations of chronic conditions.
- Manage members during short-term sub-acute rehabilitation episodes and ensure accurate and complete documentation.
- Communicate and collaborate with the interdisciplinary care team to provide patients and caregivers with counseling and education.
- Conduct advanced illness and advanced care planning conversations.
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program (NP) or accredited Physician Assistant degree program (PA).
- Active and unrestricted license in the state where you reside, or ability to obtain by start date.
- Active Nurse Practitioner or Physician Assistant certification through a national board.
- 1+ years of clinical experience in practice.
- Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice).
- Ability to complete physical requirements of the job, including lifting a 30-pound bag in and out of the car, navigating stairs, and a variety of dwelling conditions.
- Access to reliable transportation.
- Availability to work 24 hours per month, with expectations that 16 of the 24 hours/month could be during off-hours (after 5 pm, on weekends, and/or holidays) not to exceed 960 hours in a calendar year.
- Clinical experience in long-term care, skilled nursing, assisted living, acute care, ER, or Urgent care.
- Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
- Understanding of Geriatrics and Chronic Illness.
- Understanding of Advanced Illness and end-of-life discussions.
- Proficient computer skills, including the ability to document medical information with written and electronic medical records.
