Dialysis Nurse Specialist

- Educate patients on prescribed dialysis treatment and provide guidance on home therapy modality.
- Meet and evaluate prospective new home therapy patients, explaining modality options and requirements.
- Deliver high-quality, professional nursing care to dialysis patients, ensuring their safety and well-being.
- Collaborate with multidisciplinary team members to manage patient care at home, addressing daily needs and communicating effectively with patients and families.
- Provide direct and indirect patient care, utilizing nursing processes to ensure optimal outcomes.
- Facilitate referrals to other healthcare professionals and medical facilities as needed.
- Work closely with physicians and the multidisciplinary team to achieve the highest standards of care, tailored to the patient's age and developmental status.
- Ensure a secure and safe patient environment by adhering to safety policies, procedures, and standards of care.
- Offer advice and emotional support to patients and their family members, promoting a positive experience.
- Work in a fast-paced environment, frequently standing and moving around.
- Occasionally lift and move equipment weighing up to 200 pounds with assistance.
- May be required to lift chemical and water solutions up to 30 pounds and 5 feet high.
