About the Role:
As a Home Health Aide with CenterWell Home Health, you will play a vital role in providing personalized care to patients in the comfort of their own homes. Our team is dedicated to delivering high-quality, patient-centered care that addresses the physical, emotional, and social needs of our patients.
Key Responsibilities:
- Provide direct patient care under the supervision of a Registered Nurse, following a personalized care plan.
- Assist patients with self-administered medications, ensuring accurate dosing and observation.
- Take vital signs, including temperature, pulse, and blood pressure, and report any abnormalities to the supervisor.
- Help patients maintain good personal hygiene, including bathing, grooming, and skin care.
- Assist with feeding patients, communicating basic nutrition principles, and observing food and fluid intake.
- Support patients with mobility, exercises, positioning, and transfers, as per the care plan.
- Document patient interactions, care rendered, and any changes or concerns, ensuring continuity of care.
- Practice infection control principles and maintain a clean, safe, and comfortable environment.
Requirements:
- High school diploma or equivalent.
- Completion of a Certified Nursing Assistant or Certified Home Health Aide program within the last 24 months.
- Meet applicable state certification requirements.
- A valid driver's license, auto insurance, and reliable transportation.
- Good standing on the HHA Registry (if applicable) and completion of HHA/CNA course.
- At least one year of experience as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, or home health/hospice agency.
