Job Title: Clinical Autism Care Coordinator

A competitive salary of $38-44 per hour awaits you in this exciting opportunity as a Clinical Autism Care Coordinator with Teema Staffing. As a key member of our team, you will play a vital role in delivering exceptional care to individuals and families affected by autism spectrum disorder (ASD).

About the Role:
In this position, you will work collaboratively with patients, families, and healthcare providers to assess, plan, facilitate, coordinate, and advocate for high-quality, cost-effective care tailored to each individual's unique needs. Your expertise in case management and strong communication skills will enable you to navigate complex care plans, address barriers to care, and provide guidance on benefits, enrollment, and community resources.

Key Responsibilities:
  • Conduct comprehensive case management assessments and develop individualized care plans in collaboration with beneficiaries and providers.
  • Monitor and update care plans regularly to address newly identified needs, services, and treatment options.
  • Serve as an advocate for families, ensuring access to specialized services and maintaining patient safety.
  • Provide guidance on benefits, enrollment, and community resources to help families navigate available services.
  • Participate in case evaluations involving sensitive or high-profile issues and present cases to medical leadership for resolution.
  • Coordinate and lead interdisciplinary team meetings with civilian, military, and specialty care providers.

Required Qualifications:
  • Current, unrestricted licensure as a Registered Nurse or Licensed Clinical Social Worker (LCSW).
  • At least 3 years of clinical experience in pediatrics, behavioral health, ASD, or case management.
  • Eligibility to obtain a case management certification within four years of hire.
  • U.S. Citizenship and ability to pass a Department of Defense background investigation.
