Patient Care Responsibilities:

- Actively participate in daily team huddles to enhance communication and care delivery.
- Support the Regional Medical Director and other leaders in fostering a cooperative practice environment.

- Explore opportunities for service growth within the healthcare center.

Qualifications:
- A minimum of two to five years of relevant experience is preferred.
- Successful completion of an accredited Nurse Practitioner program with Board Certification.
- Active, unrestricted Nurse Practitioner license in the state of practice.
- Strong communication skills, both written and verbal, with a focus on integrated team-based care.
- Adaptability to a value-based care environment.
- Commitment to superior patient service and care.
- Basic proficiency in computer skills, including electronic medical records.
- Certification to prescribe medications under the supervision of a collaborating physician.
Preferred Qualifications:
- Active DEA license.
- Medicare and Medicaid Provider Numbers.
- Experience managing a Medicare Advantage patient panel, with knowledge of best practices in coordinated care.
- Familiarity with Medicare guidelines and HEDIS quality indicators.
- Bilingual skills are advantageous.
