Position Overview

Enhabit Home Health & Hospice is looking for a qualified Registered Nurse (RN) or Physical Therapist (PT) to take on the role of Patient Care Transition Specialist.

Key Responsibilities:
  • Guide patients through the complexities of post-acute care.
  • Evaluate, plan, implement, coordinate, monitor, and assess options and services to ensure a safe transition from acute care to home health or hospice services.
  • Utilize evidence-based clinical guidelines and protocols to develop patient-centered transition plans that enhance quality and efficiency in post-acute care delivery.
  • Encourage adherence to post-acute care plans and ensure the completion of ordered services.
  • Act as a representative of Enhabit in transitional care initiatives and build strategic partnerships with healthcare systems, hospitals, and physician groups.
  • Oversee the execution of transitional care services through ongoing quality assurance visits with referral sources.
  • Achieve and surpass referral and admission targets.
  • Serve as a clinical liaison responsible for managing care transition program admissions while positively influencing patient outcomes and referral source satisfaction.
Ideal Candidate Profile:

The successful candidate will be a goal-oriented Registered Nurse (RN) or Physical Therapist (PT) with a background in home health or hospice care.

  • Must possess a degree from an accredited nursing or therapy program and hold a valid license in the state of employment.
  • Minimum of 2-3 years of relevant field experience is required.
  • Strong knowledge of customer and market dynamics, along with transitional care best practices.
  • Familiarity with Federal, State, and local regulations governing home health and hospice operations.
  • Exceptional communication skills with the ability to engage effectively with diverse individuals.
  • Experience in territory management, strong presentation abilities, and a focus on performance management and relationship building.
  • Self-motivated individual who can work independently with minimal supervision.
  • Previous experience in home health or healthcare sales is preferred.
